There are some laser therapy products on the market that contain LED’s (Light Emitting Diodes). These products usually claim to provide superior treatment results and effects than those achieved with lasers. Whilst monochromatic, non-coherent light is useful for treating superficial wounds, research studies over the years show that lasers are more effective in treating this condition. This is due to the unique properties associated with laser light, resulting in deeper penetration depths. Many LEDs are low powered, say 5mW-15mW, with the result that energy is scattered, thus requiring longer treatment times. LED’s normally penetrate less than 1cm into muscle tissue, whilst infrared lasers, eg 830nm, penetrate up to 5cm. This proves once again that lasers are the best form of light therapy available on the market.

The Laser 3000 comes in both Continuous Wave and 7 Pulsed Nogier frequencies.

Continuous wave lasers quickly act on inflammation, stimulating blood flow and lymphatic circulation. This in turn, induces a quick reabsorption of fluid build-ups. Continuous lasers only have a secondary effect on pain, this is diminished after reducing the inflammatory process.
Pulsed lasers have an immediate impact on pain, as they are able to produce analgesia, interfering with the transmission of the pain impulses to the brain centers. These lasers are less effective at treating inflammation and edema, with results only achieved over long periods of time.

Nogier Frequencies
1. (Frequency F, 73 Hz) For use when cellular activity is hypoactive, such as chronic recurring problems, nonunion fractures and chronic splints and for stimulation of osteoid. It is also helpful in activating humoral and endocrine functions. Studies show setting 73Hz helpful in stimulating (tonifying) acupuncture and trigger points and increasing circulation in areas being treated, such as wounds when past the acute stage.
2. (Frequency G, 146 Hz) For areas of yellow scar tissue that are generally formed internally on tendons, ligaments and sub-acute (lingering but not chronic) conditions. Studies show setting 146Hz is helpful in reducing inflammation associated with injuries and infections. This is often called the universal frequency because most problems involve inflammation.

3. (Frequency A, 292 Hz.) For tissue of ectodermal origin, such as body openings, skin and nerve. Field applications include wounds, after surgery. 292Hz tends to tone tissue while minimizing the chance of hemorrhaging fresh wounds or recent surgical sites. It is also good for the treatment of acupuncture and trigger points, corneal ulcers and ulcerated mucous membranes. This is called the universal frequency in acupuncture.

4. (Frequency B, 584 Hz.) This frequency is most effective for circulatory and lymphatic stimulation and treatment of tissue of endodermal origin, such as GI tract, liver and pancreas. In field applications, 584Hz has been used in conjunction with 1168 and 146Hz for tendon, ligament, joint and other injuries where reaching secondary levels of tissue is needed.

5. (Frequency C, 1168 Hz.) For tissue of mesodermal origin, such as bone, joints, ligament, viscera and tendon. Studies show 1168Hz is especially good for tendon and ligament injuries when used with 584Hz and 146Hz. It also helps in relaxing large muscle groups.

6.(Frequency D, 2336 Hz.) For chronic conditions not responsive to setting 292Hz or 1168Hz. Studies show 2336Hz to be a good supplement to 292Hz when healing processes appear to reach a plateau.

7. (Frequency E, 4672 Hz.) For pain control, primarily when C nerve fibers are transmitting to dorsal root ganglia and when involvement of neurotransmitters is of physiological importance. Field experience shows 4672Hz to help suppress pain and to sedate acupuncture and trigger points and assist diminishing excess calcification associated with chips, spurs and arthritic conditions.

Information about 830nm wavelengths

Laser therapy reduces acute inflammation of soft tissue injuries. LLLT reduces the time from onset of injury, speeding the healing process. When used immedialtely after injury, inflammation and pain are minimized.

LLLT effectively reduces pain by increasing b-Endorphins that heal damaged muscle tissue. Low level lasers quicken the healing process. By reducing inflammation this allows the body’s natural ability to heal itself. Muscle renewal is significantly increased when treated with LLLT. The tensile strength healed tissue is also increased through LLLT. The time from the start of injury until it is healed is also reduced.

Research indicates that biological reactions are produced by the 830nm wavelengths that are not produced by other wavelengths. The shape of the laser beam emitted from the 830nm diode is egg or oval shaped. Shorter wavelengths eg visible lasers, emit a smaller, round shaped beam. Longer wavelengths, e.g. 830nm make them ideal for deep tissue penetration. The shorter wavelengths are more readily absorbed by the skin and blood, this limits their tissue penetration capabilities.

Having a longer wavelength, 830nm GaAlAs (Gallium Aluminium Arsenide) lasers are infrared and are not easily absorbed by the body, therefore they have significantly greater depths of penetration. 830nm wavelength lasers produce a larger amount of photons delivered to the treated tissue.

Increased endorphin levels are also produced with the 830nm wavelength. This wavelength stimulates the production of pain reducing plasma endorphin levels, whilst other wavelengths do not.

Studies reveal that tissue levels of ATP are significantly increased when the target area is treated with 830nm wavelength lasers. The maximum level of photons required for effective treatment is produced at this wavelength. 830nm penetrate up to 5cm, whilst other wavelengths e.g. visible red penetrate 1cm. (See below diagram, previous post).

Due to its deep treatment properties, tendons, muscle tissues and ligaments can be easily and effectively treated with 830nm wavelengths. This is not possible with short wavelengths as their penetration depths are restrictive for these conditions.

Research indicates that 830nm has the least surface absorption and thus the deepest penetration capabilities. For deep tissue injuries, 830nm is the best wavelength. At this wavelength, photons penetrate deep into the muscle tissue and treatment times are reasonable.

For treating deep lying injuries, 830nm wavelength is the most therapeutically appropriate one to choose.

Case Study

Sufficient, a racehorse owned by the Wallace Group in New Zealand was recently treated for a superficial ligament injury to its front right leg with the Laser 3000. After a treatment program suggested by Paula Stone from Stone Group Laser Technologies, the horse showed an amazing recovery, verified with scans taken by Veterinarians. All parties concerned are very impressed with the performance of the laser and the racehorse's recovery. The treatment program lasted several weeks and consisted of using a single 808nm 300mW head as well as the 658nm 90mW cluster head to the effected area.

Horse treated with Laser 3000 makes excellent recovery!

A well known racehorse recently treated with the Laser 3000 has made an excellent recovery. A scan recently taken of the injured area revealed that the damaged tendons have recovered. The horse is now fit again for racing.

Treatments were initially conducted twice per day and then once per day towards the end of the 6 week period.

The vet and owners were very impressed with the preformance of the Laser 3000.

Clinical trial on horse with injured hock!

Laserex's New Zealand Distributor for the Laser 3000, Paula Stone, has organised a clinical trial of the Laser 3000 on a horse with an injured hock.

The two year old event horse has a degenerative condition of the hock. One of his hocks is fused giving the horse pain whilst his other hock is taking a lot of pressure causing lameness.

Laser therapy, which uses an intense beam of light, stimulates the body's processes, activates waste removal, increases repair activity, relieves swelling, heals surface wounds, and stimulates blood and lymphatic systems. It also increases serotonin, thereby achieving a calming response. "Laser is especially useful," says Spalding, "to enhance the repair process in tendon and ligament injuries, superficial joint and bone injuries (including fractures), open and post-surgical wounds and ulcers, old fibrous injuries, bursitis, osteoarthritis, and rheumatoid arthritis. Laser has also been shown to be effective when used over acupuncture points."

Notes Snyder, "Therapeutic lasers seem to be very good on open wounds that are not healing well; lasers are very good at promoting and stimulating healing."

Lavallée says that physiotherapists mostly work with cold (low-intensity) lasers. "This is ideal for acute cases as it does not increase tissue temperature, increase bleeding, or aggravate the inflammatory phase."

LIGHT THERAPY AT A GLANCE

Light is energy in the form of radiation.

Therapeutic photo energy utilizes wavelengths that are found in the infrared region.

Infrared wavelengths are longer than visible red waves and shorter than microwaves. They are considered therapeutic because they are absorbed by components in tissue and blood cells and stimulate normal cell activity.

The photon is the energy packet of the light wave and the catalyst for increased cell activity.
Laser light is monochromatic, or light of one wavelength.

Laser light is coherent, meaning that the photons, or energy packets, in the light beam move in an organized wave formation.

Laser light is collimated, meaning that the light does not spread into a wider and wider beam as it moves farther away from the source.

Power output has a great influence on the effectiveness of photon therapy. Too little power will create no effect and too much power can cause a disruption in cell function - even cell death.
Power is a function of the pulse width (number of nanoseconds the pulse is on) and frequency (number of pulses per second).

Molecules are said to be wavelength specific because they absorb only the energy that matches the energy needed by the electrons they contain.

Benefits of Laser Therapy

Significant research has been conducted over the years into the efficacy of laser therapy. In fact, there are over 2000 double-blind studies that have been carried out. A summary of the most important benefits follows:

1. Increased Blood Flow by means of capillary and widening of the lumen of blood vessels, vasodilatation. Dr Toshiaki Kami showed that laser light significantly increases the formation of new capillary paths in areas treated with laser, thus resulting in increased circulation. (See Dr Kami, Effects of Low Power Diode Lasers, Annals of plastic surgery, Vol 14, No.3, March 1985).

2. Wound healing is significantly improved by low level laser therapy due to the increased levels of collagen from the laser treatment. This applies also to non-healing wounds. Collagen is the chief component of connective tissue, recognized as the most important ingredient in wound healing.

3. Laser light stimulates release of the body’s own killing chemicals such as endorphins and enkephalins. This in turn facilitates natural, long lasting pain relief.

Dr. Wolfgang Rauermeister, Director of Pain Control Center at Parkwood Hospital, Massachusetts, USA provides an excellent explanation of these pain reduction capabilities:

“The laser light stimulates the release of the body’s own pain killing chemicals such as endorphins and enkephalins from the brain, adrenal glands, and other areas, thus facilitating long lasting pain relief.”

4. Reduction in swelling. Marked improvement in the re-establishment of the lymphatic system after trauma, resulting in reduction of swelling. This is due to the removal of dirty proteins by the regenerated lymphatic vessels.

Which wavelength is best for laser therapy?

A great deal of research has been written about the optimal wavelength to use for laser therapy. The overwhelming majority of research conducted indicates that 830nm is one of the best wavelengths to use for treating deep into muscles and tissue as found on horses and greyhounds.

This wavelength has been shown to deliver the maximum amount of photons, penetrating the deepest into muscles, approximately 50mm as well as increasing endorphin levels. Laser light with an 830nm wavelength is not easily absorbed by muscle tissue, this results in greater penetration and healing of the damaged area.

To create healing of damaged tissue, it is important that the correct amount of photons be delivered to the muscle tissue. The combination of the correct wavelength and output power in combination enable the sufficient quantity of photons to be produced, penetrating deep into the muscle tissue, reducing inflammation and providing fast pain relief.

Laser therapy increases the body's production of endorphin levels. These endorphins are naturally occurring substances that help relieve pain. The Laser 3000 can come in either a single or cluster head version with 830nm wavelength. The level of endorphins produced make the 830nm version of the Laser 3000 ideal for treating painful conditions in horses and greyhounds.

830nm is well known for speeding up Adenosine triphosphate (ATP) production. This chemical produced by cells provides most of the energy used in cellular processes. The 830nm wavelength produces more energy that assists with the healing of damaged tissue, muscles, ligaments, tendons etc.
